This is hard to determine because it's hard to see if McMurphy is really sane or not. Everyone has a different side to themselves that they don't necessarily put forth out in public. So yes, I believe there is more to McMurphy than we know as readers.

McMurphy is sane, there is absolutely no doubt about that, in my mind. His laugh is a symbol for sanity, and he is a perfect foil to Harding. And with Harding as the leader of the "bull goose loonies," McMurphy represents a lone beacon of sanity. As for another side of McMurphy's personality... he was labeled a psychopath, a sociopath, and if that is at all true he would then have a myriad of other sides, all centered around his desires. As for his tough-guy attitude, it seems to be the real deal, not to say he might not have another side to him, because he definitely does, but his tough-guy attitude seems to be his foundation. And remember he was deemed psychotic because he was overzealous when it came to fighting and fornication, but we have yet to see either of those sides. Finally, his tattoo "Aces & Eights" a symbol for one of the worst possible hands a gambler can get. The tattoo could mean any of many things; he is an underdog, he wants people off-guard, or he is simply unlucky. The tattoo might even foreshadow what will happen to him, maybe he will lose he sanity and the others will "win" their lucidity back.

I beleive McMurphy does have a side that is not obvious to the reader. Underneath his tough guy persona, it seems to me like McMurphy does feel bad for his peers in the institution that are being decieved by Nurse Ratched. Because he feels badly for them, he tries to build up their confidence by telling jokes and getting them to laugh. It may not be conventional, but i think this is McMurphys way of counteracting the nurses negativity.

McMurphy is sane, even though other people in the hospital think otherwise. I think we will enentually see more of his side later in the book. The tattoo i think is an act, and trying to show that he is tough, even though that isnt what he really portrays.
